salary differential JOB SUMMARY The Skills Trainer I coordinates
with the program team members to provide community based,
comprehensive, strength-based, culturally competent, trauma
informed, flexible, effective, mental health services and supports
to eligible at-risk youth, and families. The Skills Trainer I
provides services in accordance with County Mental Health and
Family Centered Practice principles, including family voice and
choice provided in collaboration with those individuals and
agencies involved with the youth. Supports the Mission, Vision, and
Values of the agency. QUALIFICATIONS Education & Experience
Bachelor's degree in a related field AND at least 4 years of
approved experience* OR Associates degree AND a minimum of 6 years
of approved experience* *Approved experience is experience that
qualifies the applicant for Mental Health Rehabilitation Specialist
(MHRS) status in the County Children's Mental Health System of
Care. Graduate education may be substituted for the experience on a
year for year basis. Skills & Requirements Prepared to work
independently with flexible hours that may include evenings,
weekends and holidays Proven documentation skills and intermediate
level computer skills including all MS Office programs Bilingual
skills are a plus Verbal and written ability to communicate
effectively with colleagues, community partners, consumers and all
levels of management Ability to multi-task and prioritize projects
and assignments effectively Ability to work cooperatively in a team
environment within program/department and agency wide Ability to
work in a mobile, fast paced environment Ability to use a laptop
computer Fulfillment of TB test, background check clearance, and
any other mandatory State/Federal requirements Valid California
driver's license, proof of automobile insurance, can maintain a
driving record acceptable to the agency's insurance carrier, if
applicable to position Must provide and operate a registered,
properly insured automobile, if applicable to position ESSENTIAL
JOB FUNCTIONS Position Specific Provides services based on clinical
need and in consultation with supervisor to assigned youth, and
families in the home, office, school, or other community based
settings. Possesses specific cultural competence to work with and
outreach to culturally diverse populations and communities
Accountable for independent time management Provides services to
youth involved in the identified systems meeting all County
requirements as listed in program scope Provides Intensive Home
Based Services (IHBS) and/or individual rehabilitation to youth as
identified in youth's treatment plan Assists Clinician in creating
Treatment Plans and Functional Assessment/Crisis plans as a tool
for planning/ providing rehabilitation or IHBS as assigned Develops
and facilitates rehabilitative and/or skill building groups
Provides collateral support to caregivers and significant support
persons as identified in youth's treatment plan Assists Clinician
in creating individualized safety plans in coordination with
youth's child family team, maximizing the use of natural supports
and community based resources. Provides other Intensive Care
Coordination (ICC) and/or case management functions as necessary,
securing formal resources, and keeping team members informed of
progress and challenges presented. Identifies temporary and
sustainable resources and services within family and community
environments Participates in and facilitates Child Family Team
(CFT), multi-disciplinary team, hospital, crisis, psychiatric,
court, school, and provider meetings as scheduled Provides and
documents all Medi-Cal eligible billable and non-billable services,
including but not limited to plan development, group session,
collateral, rehabilitation, In Home Based Services (IHBS) case
management, Intensive Care Coordination (ICC) and crisis
intervention within required timelines Writes clear, accurate and
timely progress notes consistent with each contact/visit Seeks out
and addresses clinical feedback and recommendations provided by
supervisors Attends all staff meetings, consumer meetings, and
consultation meetings as scheduled Participates in the on-call
rotation Able to provide or arrange for safe transportation for
youth and families to services as needed Participates in
utilization, accreditation, peer review process as assigned Works
collaboratively with all internal programs, departments, and staff
to provide support as needed Maintains positive relationships with
consumers and customers Model and coach positive, constructive
attitudes and behaviors in managing difficult situations in school,
work, and living situations Develops and maintains positive
relationships with agency and community partners Responsible for
meeting all outcomes as documented in Employee Development Plan
Coordinates with other services provided under the Medi-Cal model,
acting in collaboration with other partners and ensuring
documentation reflects standards that meet or surpass agency, state
and federal Medi-Cal/Medicaid standards Agency Specific Performs
all duties in a manner consistent with the principles and values of
agency, while adhering to applicable professional codes of ethics,
the agency's policies and procedures, contractor requirements, and
regulatory requirements. Model and communicate appropriate positive
attitudes toward the agency's Mission, Vision, and Values Work
collaboratively with all agency programs and staff to provide
support as needed Utilize and maintain calendar with all
work-related details in order to manage time effectively and share
calendar information with coworkers Participate in on-going
training to expand and develop professional skills Perform other
duties as necessary for the agency, as assigned Employment Type:
Full Time Salary: $49,920 - $65,880 Annual Bonus/Commission: No
