Job Description & Requirements - Specialty: Home Health - Discipline: RN - Start Date: 04/06/2026 - Duration: 13 weeks - 40 hours per week - Shift: 8 hours, days - Employment Type: Travel Position Details: • Job Title: Home Health Registered Nurse (RN) – Palliative Care • Facility: Martinez Hacienda Medical Offices – Palliative Care Nursing • Care Setting: Home Health / Palliative Care • Location: 200 Muir Rd, Martinez, CA 94553 • Contract Duration: 13 Weeks • Start Date: 04/06/2026 • Shift: Day Shift – 8 Hours • Schedule: Monday – Friday (08:00 AM – 05:00 PM) • Guaranteed Hours: 40 Hours per Week • Work Location: Martinez Hacienda Medical Offices – 200 Muir Rd, Martinez, CA 94553 Requirements: • Active California Registered Nurse (RN) License • BLS Certification • Valid Driver’s License with Clean Driving Record • Minimum 1 year of recent Home Health experience (within the last 3 years) Additional Information: • Schedule includes 3 days of patient visits and 2 days of phone triage. • Clinic-based charting system (No OASIS required). • Coverage area includes Martinez and surrounding communities.
