Description Social Worker MSW in Utilization Review Unit at
Queen of the Valley Medical Center, CA. This position is Full time

at the direction of the Case Management Director, the Inpatient
Social Worker/Patient Counselor is responsible for assisting
patients in meeting their bio-psychosocial needs while enabling
them to access and utilize medical care and community resources in
order to achieve their optimal level of health. The Social
Worker/Patient Counselor performs crisis intervention and
short-term counseling for patients and families and transitions
patients along the continuum of care. The Social Worker/Patient
Counselor performs discharge planning for identified patients by
coordinating internal and external resources in collaboration with
the patient, family, and health care team. Providence caregivers

them. Required Qualifications: - Master's Degree in Social Work or
Counseling Psychology. - One (1) year experience as a Social Worker
or Counselor. Preferred Qualification: - Two (2) years Hospital
