We partner with PACE (Program of All-Inclusive Care for the Elderly) to help these programs care for frail seniors. The PACE model is centered on the belief that it is better for the well-being of seniors with chronic care needs and their families to be served in their community whenever possible.---

CareLinx by Sharecare helps PACE programs with healthcare professional staffing and in-home care services including transportation to medical appointments, recreational activities, and other senior care services.------


Job Summary:
CareLinx by Sharecare is looking for Per Diem Caregivers (CGs) ---to join our team. This role involves Non-medical in-home personal care as well as Non-medical appointment companionship that is decided by the client's team, hours are controlled and assessed by the client's team.

Qualifications:

• 
  
• Minimum of 6 months of experience with documented experience working with frail elderly population highly preferred
  
• Experience in a long-term care facility, companionship, and personal care experience, or with a community-based geriatric program preferred
  
• Knowledge of the principles and techniques involved in relating to frail elderly persons
  
• Skill in communicating with participants, their families, and other staff members; ability to establish a helping, trusting relationship with participants
  
• Demonstrates sensitivity to the needs of older adults as evidenced by a caring approach, patience in providing care, and responsiveness to participant requests
  
• Ability to follow directions, verbal and written instructions, and adapt work schedule under supervision
  
• Ability to utilize appropriate safety measures when providing care
  
• Ability to relate to the public and governmental agencies in a pleasant, patient, and professional manner
  
• Well organized, dependable, flexible, and resourceful
  
• Trained with First Aid and CPR
  
• Trained in Emergency Protocols and Procedure
  
• Requires the use of a personal vehicle
